Waiver of Liability & Release
(This Waiver of Liability & Release Must be Executed Prior to Participating in Open Agility)

In consideration of the foregoing, I, for myself, my heirs, executors, administrators, personal representatives, successors and assigns, waive and
release any and all rights, claims and courses of action I have or may have against Lone Star Pet Lodges of Missouri City (the "Company") and its affiliates, their agents, employees, officers, directors, managers, successors and assigns that may arise as a result of my use of the Company's training area and/or training equipment. I attest and verify that I am physically fit and have sufficiently trained for the use of the training area and/or training equipment
and my physical condition has been verified by a licensed medical doctor.

Further, I hereby grant full permission to any and all of the foregoing to use any photographs, motion pictures, recordings, or any other record of my participation in Open Agility for any legitimate purpose including commercial advertising.

I recognize that I am financially responsible for damage to any equipment caused by me or my dog. Further, I recognize that use and availability of Agility Equipment is subject to equipment availability based on on-going Agility Training classes and other Open Agility participants.

In case of inclement weather there will be no refunds.

to learn more about the great sport of Dog Agility, you can contact
our Missouri City lodge at (281) 403-7700 or send an e-mail with your
question or information to lsplmocity@lonestarpetlodges.com and someone will respond to you during our normal business hours.

For your convenience, our Missouri City Lodge is open from 7:00AM to 7:00PM seven days-a-week (except New Year's Day, Easter Sunday, Memorial Day, July 4th, Labor Day, Thanksgiving Day and Christmas Day) when we are busy caring for our holiday overnight boarding guests but not open to the public.
